Goal: To help you reach heightened levels of clarity, energy, courage, productivity, and influence
on your march to your greatest potential and performance.

By enrolling in this High Performance coaching program, I agree to the following:

I am committed to improving my life and will participate fully in each of my coaching sessions.

I am registering for 16 weeks of private, one-on-one coaching sessions and 16 weekly group coaching sessions with my coach.

I can also choose to extend the length of my program or renew at the end of my package.

I will schedule my coaching sessions on my coach's calendar through the Life On Purpose portal (https://www.lifeonpurpose.coach/booking-calendar/high-performance-1-1-online-coaching) or by phone with my coach (​201.485.6386).

I will initiate all scheduled calls and will call my coach at the scheduled time and do my best to be on-time every time. I understand if I miss a session and don’t provide 48 hours notice that I will still be billed regularly and the session I missed will still be deducted from my package.

I understand that this coaching program is about improving my present life and advancing me more quickly to my ideal future. I understand that this level of personal development coaching is not therapy, not about rehashing my past over and over, and not about professional legal, medical, psychological, or financial advice. I understand the coaching is focused on what I can do now and in the future to experience more energy, success, and fulfillment in life.

I understand and agree that my results in life are up to me and that by law my coach cannot guarantee my results in life or business, only my satisfaction, so my coach cannot be held liable under any circumstances for my results or actions in life. I understand my coach does, however, guarantee my satisfaction with the first session and that if I am not satisfied with session one, I can ask for a refund. I also understand I can stop the coaching sessions at any point I wish and I will no longer be billed for future sessions, but that I cannot be reimbursed for previous sessions after the start of the second session.

I understand that at the end of my coaching sessions I will have the option of continuing my momentum and mastery and scheduling  more sessions with my coach. It’s up to me if I continue, and if I do, I will re-enroll at my coach's current rate unless I request to stop my sessions. I can stop my sessions at any time I want.

I understand that I am responsible to take ownership over my growth and will be fully present for each call, taking notes as needed to ensure I absorb the breakthroughs and insights gained in each session. In addition, I may also receive email summaries and High Performance tools from my coach to help me advance even more quickly on my journey to High Performance.

CONFIDENTIALITY:

This coaching relationship, as well as all information (documented or verbal) that I share with my coach as part of this relationship, is bound by the principles of confidentiality set forth in the BCC Code of Ethics. However, please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. My coach agrees not to disclose any information pertaining to me without the my written consent.

 My coach will not disclose my name as a reference without my consent. Confidential Information does not include information that: 

(a) was in my coach’s possession prior to its being furnished by the Client;
(b) is generally known to the public or in my industry;
(c) is obtained by my coach from a third party, without breach of any obligation to me;
(d) is independently developed by my coach without use of or reference to my confidential information; or
(e) my coach is required by statute, lawfully issued subpoena, or by court order to disclose;
(f) is disclosed to my coach and as a result of such disclosure my coach reasonably believes there to be an imminent or likely risk of danger or harm to myself or others; and
(g) involves illegal activity.


I also acknowledge my continuing obligation to raise any confidentiality questions or concerns with my coach in a timely manner.  My coach agrees to maintain the ethics and standards of behavior established by the Center for Credentialing and Education “(CCE)” (https://www.cce-global.org/Assets/Ethics/BCCcodeofethics.pdf).

 I agree to review the BCC Code of Ethics and the applicable standards of behavior.

​​DISPUTE RESOLUTION:

If a dispute arises, I agree to notify my coach as soon as possible. If the dispute cannot be resolved by mutual consent, it will be arbitrated by telephone and both will be bound by the decision of a mutually-agreed arbitrator.

 LIMITED LIABILITY:

Except as expressly provided in this Agreement, my coach makes no guarantees, representations or warranties of any kind or nature, express or implied with respect to the coaching services negotiated, agreed upon and rendered. In no event shall my coach be liable to me for any indirect, consequential or special damages. Notwithstanding any damages that I may incur, my coach's entire liability under this Agreement, and my exclusive remedy, shall be limited to the amount paid by me to my coach for coaching services under this Agreement.

 If there is ever any disagreement that cannot be resolved, it will be arbitrated by telephone and both will be bound by the decision of a mutually-agreed arbitrator.
